Care Wishes: the current/active advance-directive survey app. Shorter survey: an essentials pass (8 pages) before everything else, several questions per page, and tone-setting questions that pre-fill suggested answers the person confirms. Covers life support, pain, where to receive care, agents, joy, after-death wishes, memory changes. Replaces the earlier version, now kept as "Care Wishes OG".
